Building a form to use the API

Hi Guys
I need a bit of help, looked at the API and the code snipit http://snipt.org/vhfha3
How do I put it together, is it a .htnl or .php page
Could you do a complete sample page (it only needs name, email submit button ) that I can then custimise it.
